We will match the young person's needs to a relevant link-worker to achieve set goals. The link worker will be the first point of contact the young person can use for whatever support is needed. We believe the initial assessment, local authority recommendations, and progress observed throughout the Leaving Care transition will determine qualitative as well as quantitative levels of support required.

Everyday needs will be catered for as a standard requirement initially and the number of visits and time will be determined on a case-by-case basis, depending on age and abilities of the young person (variations may occur as and when needs arise, for example, accompaniment to appointments) The young person's link worker will act as an advocate as well as a mentor to the young person, and some of the link worker's duties will include: Providing advice and support. Drawing up the Pathway Plan and ensuring it addresses any changing needs. Keeping in touch with the young person and the after care worker, coordinating services, and linking in with other agencies

We will help support you through work placements and apprenticeships and help you build your career path. Before your 18th birthday we will support you through your housing application and accessing benefits.

We will support you through educational pursuits and promote positive relationships. We will help identify courses that will meet with your desired need and expertise. We will promote continued educational support throughout your placement with us.

We will encourage outdoor pursuits where possible so you engage in making positive relationships with peers and within your community. We will promote regular visits for your health checks, etc.

Outcomes

We at Care Vault endeavour to champion the welfare of young people through holding management of child protection issues using the guidelines laid down in government statutes, most particularly Every Child Matters. The five outcomes are universal ambitions for children and young people, whatever their backgrounds or circumstances. They provide a framework for measuring how circumstances are improving for children and young people.
